SUPPLIER REQUEST

We are evaluating your component for a robot intended for sale in the United
States. Please provide the following for the exact configuration quoted:

1. Product name, part number, hardware revision and data sheet.
2. Actual producer's full legal name, location, and relevant parent or affiliate
   relationships. Identify the distributor separately.
3. For purchased boards and modules, the entities responsible for design and
   assembly, and the installed chips and modules relevant to our FCC review.
4. Manufacturing locations and available evidence of component origin. Where
   needed for a domestic component cost review, please identify the costs and
   origin information you can supply.
5. Applicable FCC IDs and grants, conformity information and test reports.
   Include the installation instructions and conditions for approved modules.
6. Any claimed Covered List exception or conditional approval, with the official
   record and exact models or configurations it covers.
7. Approved substitutes and how you will notify us before changing components,
   producers, production locations or the supplied configuration.
8. A technical contact who can resolve follow-up questions.

Please identify the document and revision supporting each answer. Let us know
which items require follow-up so we can take them to our reviewer.
